Service-specific visibility
Review benefits in the context of the care being delivered, rather than relying only on plan status.
Bring payer eligibility responses, benefit details, and account information together—so staff can resolve coverage questions before they become billing problems.
Match the patient, subscriber, payer, and planned date of service. Review returned coverage dates and plan information, then flag inactive coverage, demographic mismatches, and incomplete responses for follow-up.
Present available service-specific benefits, deductible and out-of-pocket balances, copayments, coinsurance, and benefit limitations. Highlight network, referral, and authorization questions that require additional confirmation. An active plan alone does not establish coverage for every service.
Route exceptions to Registration, Financial Clearance, or the authorization team with the payer response, service date, issue, and review status. Keep missing or unclear information visible instead of treating it as a verified benefit.
Recheck when service dates, coverage, or account information change. Retain the response history and verification time, and connect findings with Expected Reimbursement, Billing & Collections, and Capitation Reconciliation.

| Example / service | Returned information | Next step |
|---|---|---|
| ACME-101 · Office visit | Active coverage · $30 copay | Review completed |
| ACME-102 · Outpatient MRI | Active coverage · authorization unresolved | Financial Clearance |
| ACME-103 · Surgery | Subscriber mismatch · benefits unavailable | Registration review |
Deductible remaining: $750
Coinsurance: 20%
Out-of-pocket remaining: $2,400
Confirm MRI authorization, servicing-provider network status, and applicable service limitations.
Exigen preserves the response and routes the unresolved items to the responsible team. Staff document the confirmation and resolution before marking the review complete.
Registration reviews the subscriber ID, name, date of birth, and payer selection against the account record. After correcting verified information, staff submit a new inquiry. No benefit values are assumed while the response remains unresolved.
Illustrative benefits only. Eligibility and benefit responses are not a guarantee of payment; patient estimates also depend on applicable allowed amounts, plan terms, and adjudication. Authorization status may require a separate inquiry.
ACME demonstration only. Illustrative data and workflows; no live accounts, bank files, or payments are processed on this page.

Give each coverage question an accountable team, supporting evidence, and a documented resolution.
Keep verification dates and response history available throughout the revenue cycle.
